Pre-Employment Requirements, Information, and Forms

Congratulations on your selection for a position with Blanchfield Army Community Hospital. We are excited to have you become a part of the Blanchfield Team. There are a number of pre-employment requirements that must be completed before we can make the official firm job offer. Depending on your background and the position for which you've been selected, this pre-employment process may include a pre-employment Physical Examination, Urinalysis, Security Background Investigation including fingerprints, and a thorough Credentialing and Privileging determination. This process can be lengthy but we are here to assist you in any way needed. Your responsiveness in completing and submitting required documentation is very important and will expedite the hiring process. All documents will be sent to you by the onboarding team with completion and return instructions.
