The Patient-Centered Medical Homes support children's primary care needs. Air Assault Family Medical Home, Byrd Family Medical Home, and Screaming Eagle Medical Home support primary care services for all ages, both adults and children. The Young Eagle Medical Home is a pediatric-based clinic that provides the full spectrum of pediatric care to children 17 years of age and younger.

In the event your child's care team requests for ADHD forms to be completed for your child, parents are asked to have two forms completed. The Initial ADHA packet includes one form to be completed by the parent and the second form to be completed by the teacher. Please use the following links to print out ,complete the initial ADHD parent form and request your child's teacher to complete the second form.

For an ADHA follow-up visit, your child's care team requests for a Follow-up Packet be completed. Please print out the Parent and Teacher Follow-up Forms using the links below. Once the forms are completed, please ensure your child's care team receives the completed packet.
