All BACH Health System Patient Centered Medical Homes offer walk-in Flu vaccines. Air Assault, Gold, Young Eagle, and Screaming Eagle Medical Homes offer Flu vaccines Monday-Wednesday, and Friday, 8 a.m.-2:30 p.m.
Rock Island Arsenal Woodson Health Clinic offers walk-in flu vaccines Monday-Friday, 1-3:30 p.m. ages 6 months to 64 years.

Blanchfield Army Medical Homes are based on a Patient-Centered Medical Home (PCMH) model that has been adopted across Army Medicine and many civilian healthcare systems. The patient-centered, team-based and holistic concept allows us to provide you and your Family more personalized care with multiple ways to access your care team. From office visits and walk-in services to telephone consults (TCON) and secure messaging, you and your Family have even more ways to get the care you need.

This model is based on the belief that your well-being should always be at the center of every decision about your health. With this new approach, you, the patient will receive more personalized and coordinated care, supported by an entire team to maximize overall wellness. You no longer have just a primary care provider but an entire primary care team!

The ADHD Diagnostic Rating Scale is a psychological assessment tool for parents of children aged 6 to 12 designed to measure the severity of attention deficit hyperactivity disorder (ADHD) symptoms. In the event your child's care team requests for ADHD forms to be completed for your child, parents are asked to have two forms completed. The Initial ADHD packet includes one form to be completed by the parent and the second form to be completed by the teacher. Please use the following links to print out ,complete the initial ADHD parent form and request your child's teacher to complete the second form.

For an ADHD follow-up visit, your child's care team requests for a follow-up packet be completed. Please print out the Parent and Teacher follow-up forms using the links below. Once the forms are completed, please ensure your child's care team receives the completed packet.
