Blanchfield will begin surveying about 14,000 high-risk healthcare beneficiaries through an automated phone survey starting January 7 and into the next several weeks to assess their preference for receiving the COVID-19 vaccine.

At this time, no appointments are being scheduled for beneficiaries to receive the vaccine.
Beneficiaries will be alerted to when and how they can make appointments once the vaccine is available to them.

Byrd Soldier Medical Home

The Specialist Jordan M. Byrd Army Medical Home staff members provide primary medical care services to the Soldiers of 101st Combat Aviation Brigade, 101st Sustainment Brigade & Tenant Units on two separate teams, Team Lifeliner and Team Wings of Destiny. In addition, Byrd Army Medical Home provides primary care to approximately 5,000 Active Duty Family Members and Retirees.

Byrd Army Medical Home offers seven Ancillary Services and is home to the Atkins Dental Clinic. On-site ancillary services are open from 8:30 a.m. to 4:30 p.m. Monday through Friday for Active Duty personnel include Optometry, Audiology, Physical Therapy, Radiology, Laboratory, Medical Records, and Pharmacy services.

Provides hearing readiness testing and earplug fittings for active duty personnel empaneled to Byrd Army Medical Home.


All Soldier Centered Medical Homes, including Byrd, LaPointe and Campbell Army Airfield, offer immunizations for active duty Soldiers. Immunizations offered include: HPV, Shingles, tuberculosis, pneumonia, measles, mumps, rubella, Anthrax, hepatitis A and B, diphtheria, tetanus and whooping cough, meningitides, and polio. Soldiers may schedule an appointment to receive their immunizations via TOL at or BACH’s Appointment line at 270-798-4677 or 931-431-4677.
